FLOW Grappling Club - Full Liability Waiver
ACKNOWLEDGMENT AND ASSUMPTION OF RISK:
I understand that Brazilian Jiu-Jitsu and other martial arts involve inherent risks of injury, including serious injury and death. These risks may arise from training, sparring, competitions, equipment, or other participants. I acknowledge that I am voluntarily participating in these activities with full knowledge of the risks involved and I assume all such risks.
WAIVER AND RELEASE OF LIABILITY:
I hereby release FLOW Grappling Club, its owners, employees, instructors, volunteers, agents, and representatives from any and all liability for personal injury, death, property damage, or other loss sustained by me or my minor child while participating in any activities or while on the premises. This includes claims arising from negligence, to the fullest extent allowed by law.
INDEMNIFICATION:
I agree to indemnify and hold harmless FLOW Grappling Club from any claims, lawsuits, or costs (including legal fees) related to injuries or damages incurred during my participation or the participation of my minor child.
MEDICAL EMERGENCY:
I authorize FLOW Grappling Club to seek emergency medical care if needed. I understand that I am responsible for any costs incurred for medical services.
MEDIA RELEASE:
I grant FLOW Grappling Club permission to use photos or videos of me (or my child) taken during training, classes, or events for promotional and marketing purposes without compensation.
PARENT/GUARDIAN CONSENT (if applicable):
I certify that I am the legal guardian of the listed minor and have the authority to sign this waiver. I agree to all the terms above on behalf of my child and understand this waiver is binding.
ENTIRE AGREEMENT:
I acknowledge this waiver is legally binding and enforceable under Texas law. I have read and understand the waiver in its entirety and agree voluntarily.
